RNAO’s work on ECCO RNAO’s Enhancing Community Care for Ontarians (ECCO) 3.0 model calls on the government and health system partners to strengthen community care and anchor the health system in primary care to be er meet the health needs of all Ontarians (RNAO, 2020a). [...] The improved access to popula on health data and personal health informa on supports be er integra on and collabora on among health providers and persons receiving care (RNAO, 2020a; RNAO, 2024a). [...] Interoperability refers to “the secure and mely exchange of health informa on between systems (e.g., health technology solu ons, devices, consumer apps) and the common interpreta on of that informa on devoid of addi onal ac on from users (Canada Health Infoway, 2023).” Canada and Ontario experience significant barriers related to sharing and accessing health data. [...] Interoperability and popula on health management Digital health technologies support integra on and collabora on among care providers and pa ents, improving access to popula on health data and personal health informa on. [...] According to the Ontario Health Data Council Report (Ontario Health Data Council, 2022), popula on health management leverages health and sociodemographic data to iden fy the unique health and social needs of defined popula on segments.
