It will allow HIN planners and operators to objectively assess themselves, and to develop plans for enhancing their operational capabilities, and the level of service and value they deliver. [...] Overall, the paper provides insights and guidance for leaders from organizations within Canadian jurisdictions and regions that are ultimately responsible for policy development to support and encourage the creation and operation of HINs, or are responsible for the build-out, deployment and operations of HINs. [...] It provides these individuals with information and tools they can use to objectively assess the current state of their HINs (and HIN stakeholders and participants), and to develop plans through which they can enhance the capability of the HINs so as to improve the overall level of access, quality, cost and productivity of health care planning and delivery. [...] A HIN, in the context of this document, is an organization which provides three key sets of core capabilities: 1. Information Exchange and Interoperability The HIN provides the facilitation and coordination of clinical information exchange among various stakeholders in a private and secure way, including transmission of information and the management of exchange among the health care stakeholders [...] HIN Capability Maturity Model Definition The HIN Capability Maturity Model comprises a set of structured levels that describe the behaviours, practices, processes, capabilities and milestone achievements which a HIN will develop over time to reliably and sustainably deliver the infrastructure and services required to support health and health care related outcomes for a province or territory.
