Through facilitated discussions, the stakeholders reflected on the findings and discussed how patients’ needs inform the design of ‘e-integrated care plans’; the importance and value of integrated care plans and enablers to support the advancement of ‘e-integrated care plans’. [...] In addition, patients expected providers to plan care with their involvement and have effective means for documenting and communicating the plan of care to the patient and between care providers. [...] In discussing the importance of ‘e-integrated care plans’, the stakeholders noted that care plans are one clear mechanism for patient empowerment and ownership of their own care including promotion of self- management and enhancing patient confidence in the health care system. [...] The above studies are difficult to compare because of the varying interventions and contexts; however, they demonstrate the challenge in understanding the specific nature of electronic care plans and the impact of care plans on health and other outcomes. [...] The main themes of the patient centered care plans were the ability for patients to view their information and to add to the care plan.
