The main objectives of the workshop were: i) to identify and rank common Adverse Events; ii) to identify and rank important research questions; and iii) to develop research networks. [...] The one day CIHR funded workshop was designed to determine what professionals in the field believe are: a) the principal Adverse Events experienced by patients who transition between homecare and acute care settings; and b) important research questions that will help address adverse events with the aim of improving quality/patient safety and preventing future Adverse Events. [...] The four main criteria identified were the following: i) degree of harm to client; ii) frequency of occurrence; iii) cost to system of the adverse event; and iv) cost and effectiveness and potential to prevent and avoid reoccurrence. [...] As part of the small group and large group exercises in identifying and reaching consensus on Adverse Events, it became clear that a better understanding of Adverse Events would also include additional research/knowledge relating to the contributing factors associated with the occurrence of Adverse Events. [...] The research team specified the research questions and objectives of the workshop to the consultant and developed the workshop protocol and agenda with the consultant.
